Access to PET Foundations from Cardinal Health

As a customer of DMS Health Technologies' mobile PET/CT services, you are also a client of Cardinal Health. This relationship grants you access to Cardinal Health's exclusive PET FoundationsSM web site full of useful information such as 's, marketing materials, reimbursement, and patient, physician, technologist and administrator information. To begin accessing these resourceful tools:

1. Log on to www.petfoundations.com. From the homepage, select "Become a Member" from the Member Log-In box.
   
   
   
2. Select "Join Now" from the PET Foundations Web Site Member section.
   
   
3. From the Sign Up for Your New Account page, begin by entering in a unique Member Id that you would like to use. Continue down the form and fill in information required to your new account.
   
   Once you have filled in the necessary information, select
   "Create My Account".
   
   
   
4. To complete the new account setup, continue to
   Submit Your Profile.
   
   Cardinal Health Customer Number: If you do not know your Cardinal Health Customer Number, you can either enter in 000000 or contact your local Cardinal Health PET Specialist. If you enter in the six 0's, Cardinal Health will fill in your specific customer number when they approve your access to the site.

After you are complete, you will see the Your account has been successfully created! screen. Once you receive an email confirmation from Cardinal Health (1-2 business days), you will have full access to the online tools, as well as the ability to update or list your PET/CT Center in Cardinal Health's exclusive PET Locator tool.
